Frequently Asked Questions

Is Carly a licensed therapist?
Yes. Carly Poloskey holds an LMHC (Licensed Mental Health Counselor) license in Florida and is also a Board-Certified Music Therapist (MT-BC).
Do you offer virtual/telehealth sessions?
Yes, sessions are available via telehealth for clients through a HIPAA-compliant video-conferencing service.
Do you offer in-person sessions?
At this time, Creative Elements is entirely virtual. We hope to offer in-person services in the near future.
Do you take insurance?
Creative Elements Health & Wellness operates on a self-pay (private pay) basis. This means services are paid for directly by the client rather than billed to an insurance company.
What This Means for You
Because sessions are not billed to insurance, a formal mental health diagnosis is not required in order to receive services. Insurance companies require a diagnosis to establish "medical necessity" before they'll reimburse for treatment — but as a self-pay client, you can engage in therapy for personal growth, specific challenges, or general wellbeing without a diagnostic label attached to your care, unless one is clinically appropriate and useful to your treatment.
Why This Practice Operates This Way
-
Privacy: Diagnoses submitted to insurance become part of a permanent record that can be accessed by the insurance company, and in some cases shared with other entities (e.g., during background checks, life insurance or disability applications, or if records are subpoenaed). Self-pay keeps that information between you and your therapist.
-
Flexibility: Not every reason for seeking therapy fits neatly into a diagnostic category. Self-pay allows treatment to be guided by your actual goals rather than by what insurance will approve or reimburse.
-
Clinical integrity: It removes any incentive to assign or emphasize a diagnosis in order to justify continued sessions to a third party.
If You Want to Seek Reimbursement from Your Insurance
Upon request, a superbill (an itemized receipt of services) can be provided so you may submit for potential out-of-network reimbursement. Please note: most insurance companies require a diagnosis code on a superbill in order to process reimbursement. If you request a superbill, a diagnosis will need to be assigned at that time, and it will become part of the record submitted to your insurer. This is entirely optional and at your discretion — you are never required to request a superbill.
How much does therapy cost?
Initial/intake session- $250
Individual therapy- $200
Couples/Family Therapy - $250
Rates can be discussed in consultation calls. I honor that every financial situation is unique. Sliding scale spots are available.
What is music therapy, and is it different from talk therapy?
Music therapy uses music-based interventions — such as songwriting, instrument exploration, and music listening — as a clinical tool alongside traditional counseling. It can be especially helpful for clients who find pure talk therapy limiting, offering another way to process emotions and experiences.
Do I need to have musical experience to try music therapy?
No musical background or skill is required. Music therapy techniques are adapted to each client's comfort level and goals — it's about using music as a tool, not about performance.
